Requesting Information From Your Doctor

When seeking a medical report from your doctor, we make sure to request the following:

  • Medical history: Your doctor should explain what your initial symptoms were and when they started. He or she should explain what was observed in the initial exam, being as specific as possible.
  • Treatment: Your doctor should detail the diagnosis and prognosis, and also explain what treatments were administered. It is also important for your doctor to detail what diagnostic tests were given.
  • Impact on your life: Your doctor should detail how your disability affects your ability to perform your job. Does a back injury prevent you from sitting or lifting? Does a brain injury affect your ability to perform your job? If your condition is disabling, your doctor should make that clear in the report.
